POSITION TITLE: Med Services QMHP

POSITION SUMMARY:

Complete initial/annual Brief evaluations on individuals in maintenance level of care. Provide community support for maintenance level needs.  Provision of intake assessment and crisis back-up.

PRIMARY RESPONSIBILITIES:

• Provide integrated community support services and interventions to Maintenance level caseload according to assessed needs, utilizing professional and independent judgment.

• Completes initial/annual Brief Evaluations for consumers in Maintenance Level of Care.

• Develop with participation of person served an integrated, person-centered treatment plan to assist consumers in achieving treatment goals.

• Complete initial, annual and discharge Adult/Youth status reports.  Develop a tracking system to ensure status reports are being completed in a timely manner.

• Complete all agency paperwork in a timely and clinically appropriate fashion.  This included referral, transition and discharge summaries and should include appropriate documentation of medical necessity.

• Serve as crisis back-up clinician as needed for crisis calls and/or intakes.

• Maintain billable expectation and complete all paperwork in adherence to agency timelines.

• Serve as a resource for treatment team and be available to staff problematic consumer issues or situations which may arise which require clinical discussion for best practice outcomes.

• In conjunction with the Eligibility Coordinator, ensure consumers are applying for and obtaining necessary entitlement.

• Utilize available IT resources/tools to supplement assessment and treatment planning including CyberAccess, Infoscriber and Avatar.

• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.

• Other duties and/or responsibilities may be assigned by supervisor.

MINIMUM QUALIFICATIONS • Master’s Degree in related field required • LPC/LCSW or CSAC I or II preferred • 1-3 years position-related experience required
