POSITION TITLE: Nurse Liaison

POSITION SUMMARY:  The primary responsibility of the Nurse Liaison will be to manage the implementation of computer-based disease management programs for DMH Net. 

RESPONSIBILITIES: (abbreviated)

• Performs job duties in accordance with the Tri-County’s vision, mission, and values

• Fifty percent of job duties will be devoted to; Coordinating DMH Net technology programs including Health Care Optimization (HCO), Treatment Adherence Program (TAP), Behavioral Pharmacy Mailings, and Cyberaccess.

• For CCIP program, reviews and completes the Plan of Care (POC), and updates monthly. Confers with CCIP Health Liaison, CPRC Manager, and/or CSWs.

• Consults with CSWs involving their clients and provides educational training on medical disease states, client coaching, and medications

• Assists in developing or enhancing a wellness initiative, such as, but not limited to, implementation of monitoring tools, wellness support groups, diet and exercise programs, weekly education, health fairs, and screenings.

• Other duties and/or responsibilities may be assigned by supervisor.

MINIMUM

QUALIFICATIONS:  

·       Bachelor’s Degree preferred

·       Missouri License

·       3-5 years of position-related experience

·       Healthcare technology software experience preferred

·       Organizational and leadership skills

·       Ability to develop educational training and programs on various topics to promote a wellness initiative within the agency

·       Self motivated and able to work autonomously
