POSITION TITLE: Transitional Case Manager

POSITION SUMMARY:
Provides temporary case management services to persons diagnosed with a serious and persistent mental illness, and to persons with co-occurring conditions. This service is provided in a community-based program, assisting the person to adjust to community living as well as managing the symptomology of mental illness. Assesses new clients to determine an individual’s needs for on-going case management services and provides those services until a permanent CSW is assigned. Manages CPRC waiting list, makes CSW assignments and liaisons with contract CSW agencies and internal CSW’s, community resources as needed for client care.

RESPONSIBILITIES:
• Assess clients referred to CPRC program to determine individual’s needs for ongoing case management services.

• Determine immediate needs, identify stage of change, make appropriate referral for service and support needs, self help groups, including those for co-occurring substance use conditions, and/or other community and agency resources.

• Manages caseload of clients waiting for permanent case manager assignment, including providing direct services to clients on waiting list for permanent case management assignment

• Assigns clients to permanent case managers as soon as possible, based on prioritization criteria. Serves as liaison with internal and external case managers to facilitate case assignment and transition of clients to permanent case management services.

• Provides crisis and assessment services as needed.

• Encourages and assists clients to establish relationships and natural supports within the community, including access to self-help resources, and assists with advocating for these supports as needed.

• Maintains required training and certifications, including credentialing in integrated dual disorders treatment.

• Maintains productivity expectations of providing 25+ billable service hours per full time work week

• Maintains knowledge of and proficiency in use of agency technology programs/systems needed to perform job functions

• Locates and coordinates services and resources to resolve client crises and needs, and assists clients to utilize community resources effectively

• Treats internal and external consumers with dignity and respect, and encourages others to do the same

• Assists and supports clients in making and carrying out health care needs decisions as needed

• Attends and participates in team, employee and individual supervision meetings

• Maintains communications with other treatment providers for each client on case load as needed to insure provision of needed services

• Discuss and provide information and other assistance to individual client and client support network, including guardians, RCF staff, family members, etc., during assessment process, waiting list time until permanent case manager assignment, and during transition to permanent case manager case load.

• Provides information to treatment staff assigned to client to facilitate on-going services

• Documents provision of services through progress notes as per agency-wide policy and procedure

• Other duties and or responsibilities may be assigned by supervisor.

MINIMUM QUALIFICATIONS • Bachelor’s degree required • QMHP preferred • Up to one year position-related experience • Ability to work independently • Strong time management skills
