HealthCare Home

HealthCare Home is an intense, focused effort to treat Tri-County clients as “whole people” by making sure that both their mental and physical health needs are met.

In less than a year, the HealthCare Home programs has met or exceeded all benchmarks established by the Missouri Division of Behavioral Health. On a human level, the  program works with nearly 1,000 consumers to screen for issues such as diabetes or high cholesterol, which otherwise may go untreated and impact the consumers’ physical and mental health.

For the community, these advances also mean better health outcomes combined with reductions in things such as emergency room visits, which often end up costing everyone.
